Please verify that your information is up to date and accurate, including your demographics, insurance details, credit card information, and any other relevant data.

 

For the other forms, simply click "Next" in the blue box at the top to cycle through them.

 

When you reach the last option labeled "Complete Check-in Process," scroll down the page and click the orange button that says "Complete Check-in Process."

 

Once you've completed this step, your telehealth appointment will automatically display on your screen. You will then be prompted to allow access to your video camera and microphone.

 

Please select "Allow," and your appointment will begin with your provider.
